Potential big news for the 40 million Americans withmigraine.

I filed a citizen petition with the FDA to make triptans —the most effective acute migraine treatment — available over the counter. Thepetition has been accepted and is now open for public comment: 👉https://www.regulations.gov/document/FDA-2026-P-0925-0001

There are over 40 million migraine sufferers in the US. Surveysshow that less than half will ever see a doctor. There is a significantshortage of neurologists and primary care physicians. There are fewer than 800board-certified headache medicine specialists nationwide. Requiring aprescription for the most effective migraine treatment means that, for the majorityof sufferers, that treatment might as well not exist.

Right now, most people with migraine self-treat with OTCpain relievers. Search "Migraine Medicine" on Amazon and the topresult is a 300-count bottle of acetaminophen/aspirin/caffeine (genericExcedrin).

You can also buy 1,000-count bottles of ibuprofen and acetaminophen.Some of my colleagues have raised concerns about medication overuse headache(MOH) with OTC triptans. I understand and share the concern about MOH — but theevidence shows that this concern is misplaced.

A systematic review (Thorlund et al., J Headache Pain2016) found that triptans carry a 35% lower relative risk of MOHcompared to analgesics. The drugs most strongly linked to MOH are combinationanalgesics — especially those containing caffeine. That 300-tablet bottle isprecisely the kind of product that drives overuse.

Beyond MOH, this combination carries real risks: GI bleedingfrom aspirin, liver toxicity from acetaminophen (especially with alcohol), andcaffeine dependency that itself fuels a rebound cycle. Meanwhile, triptans havebeen used safely by hundreds of millions of patients worldwide for over 30years and have been available OTC in the UK, Germany, and other Europeancountries for many years.

The irony is that the drugs that are most likely tocause MOH and carry the most serious side effects are freely available in bulk,while the most effective and arguably safer migraine-specific treatmentrequires a prescription that most sufferers will never obtain.
